SUMMARY:IMechE: Technical presentation on the benefits of failure investigations
DESCRIPTION:This talk will discuss what we can learn from analysing weld failures to help us avoid them in the future and prevent problems with operational plant. \nWhat will you learn? \nFailure investigation is only a small\, but essential part of the root cause analysis process.  This presentation highlights the difference between failure investigation and root cause analysis\, considers the methodology of a failure investigation\, and using examples\, covers some of the pitfalls related to interpreting the data arising from failure investigations.  Some of the challenges associated with failures which occur frequently\, or occurred due to repairs being carried out without investigation of the failure are also considered. \nOur industry expert: \nJoanna Nicholas\, Consultant Welding Engineer at TWI Ltd \nMeet the speaker:  \nJoanna joined TWI Ltd in 1999\, and has been involved in research\, materials selection\, and failure investigations\, mainly with ferritic steels\, alongside welding consultancy.  She has also been involved in various aspects of computer modelling of weld phenomena and welding software supporting standardisation of weld procedures and welder qualifications. \nJoanna has a broad knowledge of ferritic steels with particular emphasis on hydrogen cracking and hydrogen diffusion\, HAZ hardness\, mooring chains\, fasteners\, in-service welding and 9% Ni steels.
